What to Bring to the Office For Your First Visit
Remember: On the day of your first visit to Bernier Physical Therapy Associates, please arrive at least 15 minutes early so that you will have time to fill out a patient information form. We'll ask for personal and medical information needed to process your claim and ensure your phsycial health. Or feel free to download the patient information form and fill it out before you arrive.
