What To Bring to a Physical Therapy Appointment
On the day of your first visit to Bernier Physical Therapy Associates, please arrive at least 15 minutes early so that you will have time to fill out a patient information form. We'll ask for personal and medical information needed to process your claim and ensure your phsycial health. Or feel free to download the patient information form and fill it out before you arrive.
Please bring your medical insurance card so that we can file a claim with your medical insurance provider on your behalf. Co-payments are also due at the time of your visit.
You will also want to wear loose, comfortable clothing as physical therapy may require stretching and physical activity within the limits of your treatment capabilities.
