Billing Policy
It is the policy of Bernier Physical Therapy Associates, Inc. to require payment for services rendered at the time services are provided. If the patient has insurance coverage available, Bernier Physical Therapy Associates, Inc. will accept payment through an identified and confirmed insurer, with the understanding that the patient remains responsible for any co-payment or deductible. In the absence of such insurance coverage, the responsibility for payment for services rendered by Bernier Physical Therapy Associates, Inc. is the patient's.
The only exception to this policy is a situation where a patient is pursuing a claim for monetary damages as a result of an accident or injury for which Bernier Pysical Therapy Associates, Inc. is providing services. In such a situation, Bernier Physical Therapy Associates, Inc. will consider an assginment of proceeds received in resolution of the patient's claim, provided that the patient signs an Assignment Agreement, and provided further that the patient's attorney likewise signs the Assignment Agreement. Please call or email our office for a copy of the Assigment Agreement. In all other instances, payment is expected at the time of service.
Attendance Policy
Out of courtesy to other patients who are waiting to be treated, please give us at least 24 hours notice if you need to cancel your appointment, or a nominal fee will be assessed to you. However, we do understand extenuating circumstances can occur and we will waive that nominal fee after reviewing your situation.
